Personal information on this form is collected under the authority of sections 58.5(1), 265(d) and 266 of the Education Act, R.S.O. 1990, c.E2, as amended, and the Ontario Student Record (OSR): Guideline, 2000 and will be used by Toronto District School Board (“TDSB”) to process your request and retrieve your records as well as for statistical and administrative purposes related to TDSB transcript services. The Board has a duty to protect the personal information that it collects and to follow its legal obligations under the Municipal Freedom of Information and Protection of Privacy Act, R.S.0. 1990, c M56 regarding the Board's collection, use, disclosure, and retention of personal information in its custody and control. The Board collects personal information through CTO Requests for OTSR Applications and corresponding email communications. Any new purpose for the use of personal information will be documented. The amount and type of information gathered will be limited to what is necessary for the identified purpose. The information will be disclosed to authorized TDSB staff to administer the above purposes.
